That is the common case. Aerosys closed and AboveAir left the through-the-wall market, so buildings that standardised on either have no catalog replacement. For the 36 inch by 19 inch opening we build a custom R-454B unit to fit the sleeve already in the wall.

How far ahead should we order?

Further than feels necessary. Custom 36 by 19 units are built to order on a 2 to 3 week lead time, and that is before board approval, a certificate of insurance, and a service elevator booking.
